All applicants applying under this external announcement will be subject to Veterans' preference.</strong><br /><br />Learn more about this agency <br /><br /><b>Responsibilities</b><br /><br />- The Clinical Pharmacist in the Hampton VA medical center manages all Community Care Programs including the VA Mission Act of 2018 with regards to medication order review and processing.<br /> – The Clinical Pharmacist plans and coordinates pharmacy services for patients enrolled in Community Care and Community Care providers within the guidelines of the Community Care programs, Joint Commission standards, and medical center policies/regulations. <br /> – The Clinical Pharmacist is responsible for addressing a variety of questions posed by physicians, nurses, coworkers and patients related to drugs, pharmacy services and departmental policies and procedures.<br /> – He/she must be able to interpret and apply VA, federal, and state regulations, Joint Commission, DEA, FDA, hospital and Pharmacy Service polices under all circumstances.<br /> <strong>Program Management – Community Care</strong><br /> 1. Provides pharmacist oversight for Community Care programs by reviewing and entering outside prescriptions written by Community Care providers for patients enrolled at the medical center.<br /> 2. Serves as consultant and information resource to the Community Care physicians and applicable medical center staff.<br /> 3. Ensures patient prescriptions written by outside providers conform to both national and local VA formulary.<br /> <b>Program Management – Therapeutic Interchange</b><br /> 1. The Clinical Pharmacist provides clinical pharmacist services for medical center patients, caregivers, and providers when therapeutic interchanges are mandated / deemed prudent. This may be achieved either verbally or via written correspondence.<br /> 2. Assists in formulary management through facilitation of therapeutic substitutions that do not require a Scope of Practice, non-formulary reviews, medication use evaluations, and documenting/assessing adverse drug events.<br /> <b>Pharmacy Service</b><br /> 1. Reviews, interprets, and verifies medication orders for appropriateness, processes medication orders, and interacts/makes recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care.<br /> 2. Reviews patient medications, allergies, labs and other pertinent information from the medical record to identify and solve medication-related problems, contacting providers as appropriate.<br /> 3. Regularly documents recommendations and interventions.<br /> 4. Provides refill extensions and partial medication supplies when appropriate.<br /> 5. Interprets health and medication histories, performs medication reconciliation, and provides drug information.<br /> 6. Provides written and verbal (telephonic) patient education to patients. <br /> 7. Serves as pharmacy liaison and point of contact for traveling veterans with medication related issues.<br /> 8. Drafts and facilitates delivery of patient education when deemed appropriate via telephone or in written form.<br /> 9. Reviews ADRs and assists with VA ADERS completion.<br /> 10. Performs non-formulary and prior authorization drug consult review and completes said requests timely (<br /><br /><strong>Work Schedule:</strong> 7:30 AM to 4:00 PM or 8:00 AM to 4:30 PM (Monday through Friday). Today the sole degree is Doctor of Pharmacy.)<br /> (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).<br /> <b>Licensure:</b> Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.
