<b>Duties</b><br /><br /><b>Summary</b><br /><br /><strong>About the Position: </strong>The Nurse Consultant Medical Home Port Team (MHPT) and the Patient Centered Medical Home (PCMH) position is located at Fort Belvoir Community Hospital in the Directorate for Medicine. Work is performed in a hospital based ambulatory clinic, where exposure to contagious and infectious disease is common. Employees work in shifts to ensure adequate coverage during the full clinic hours.<br /><br />This is a Direct Hire Solicitation<br /> Learn more about this agency <br /><br /><b>Responsibilities</b><br /><br /><ul> <li>Serve as a team leader and member of a multidisciplinary primary care team, which includes a variety of medical personnel, such as physicians, nurse practitioners, physician assistants, nurses, case managers, etc.</li> <li>Oversee nursing care of paraprofessionals.</li> <li>Encourage patients and their family to actively participate in patient health care needs.</li> <li>Provide a variety of nursing services for pediatric and adult patients, to include telephone triage, patient assessment/evaluations, care coordination, lab draws, IV infusions, etc.</li> <li>Utilize automated programs and information technology tools to facilitate the patient experience.</li> <li>Plan and implement performance improvement activities, based on the process improvement plan for the medical treatment facility.</li> </ul> <br /><br /><b>Travel Required</b><br /><br />Not required<br /><br />Supervisory status <br /> No <br /><br />Promotion Potential <br /> None <br /><br /></li> <b>Requirements</b><br /><br /><b>Conditions of Employment</b><br /><br /><ul> <li>This position requires the incumbent be able to obtain and maintain a determination of eligibility for a Tier 3 security investigation or access for the duration of employment.</li> <li>This position requires the incumbent to have tuberculosis testing.</li> <li>Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation.</li> <li>Two year trial/probationary period may be required.</li> <li>This position has mandatory seasonal influenza vaccination requirements and is subject to annual seasonal influenza vaccinations unless otherwise exempted for medical or religious reasons (documentation of exemption must be provided upon hire).</li> <li>This position is subject to a pre-employment drug screening, and random testing thereafter, to include testing based on reasonable suspicion and testing due to direct involvement with an on-duty accident.</li> <li>This position requires the incumbent to fulfill credentialing requirements and obtain and maintain appropriate/relevant clinical privileges.</li> <li>This position requires that you obtain and maintain a Basic Life Support (BLS) certification.</li> <li>This position requires the incumbent to complete a Childcare Background Check (due to regular contact with children or adolescents).</li> <li>This position requires you to satisfactorily complete a pre-employment physical examination.</li> <li>This position may require the incumbent to work on any day or shift, including PMs, nights, weekends, holidays, or on an ?on call? basis, necessary to meet mission requirements.</li> <li>This position has been designated as Mission Essential and the incumbent must assume essential personnel status during inclement weather/emergencies.</li> </ul> <br /><br /><b>Qualifications</b><br /><br /><strong>Who May Apply: US Citizens</strong><br /><br />In order to qualify, you must meet the education and experience requirements described below. Additional information about transcripts is in this document .<br /><br /><b>Basic Requirements:</b><br /> <strong>Education: </strong>A graduate or higher level degree, bachelor's degree, associate's degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.<br /><br /><strong>Degree from Foreign Nursing School:</strong> Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.<br /><br /><strong>Licensure: </strong>Applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.<br /><br /><strong>In addition to the above requirements, applicants must meet one of the following requirements:</strong><br /><br /><b><strong>Specialized Experience:</strong></b> One year of specialized experience which includes caring for medical/surgical pediatric and adult patients; performing triage; communicating multi-disciplinary care plans; and educating patients on treatment plans.This definition of specialized experience is typical of work performed at the second lower grade/level position in the federal service (GS-09).<br /><br />OR<br /><br /><strong>Education Substitution:</strong> Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field.<br /><br /><b>Education</b><br /><br /><strong>FOREIGN EDUCATION:</strong> If you are using education completed in foreign colleges or universities to meet the qualification requirements, you must show the education credentials have been evaluated by a private organization that specializes in interpretation of foreign education programs and such education has been deemed equivalent to that gained in an accredited U.S. education program; or full credit has been given for the courses at a U.S. accredited college or university.
